OF ENVIRONMENTAL REVIEW

PROJECT: Lakeville Hospital Redevelopment

LOCATION: 43 Main Street, Lakeville, MA 02347

The undersigned is submitting an Environmental Notification Form (“ENF”) to the Secretary of Energy & Environmental Affairs on or before

April 1st, 2024

This will initiate review of the above project pursuant to the Massachusetts Environmental Policy Act (“MEPA,” M.G.L. c. 30, ss. 61-62L). Copies of the ENF may be obtained from: Michael Karasik at mkarasik@vhb.com or 617.607.2936

Electronic copies of the ENF are also being sent to the Conservation Commission and Planning Boards of the Town of Lakeville, and a hard copy has been made available for public viewing at the Lakeville Public Library.

The Secretary of Energy & Environmental Affairs will publish notice of the ENF in the MEPA Environmental Monitor, receive public comments on the project, and then decide if an Environmental Impact Report is required. A site visit and/or remote consultation session on the project may also be scheduled. All persons wishing to comment on the project, or to be notified of a site visit and/or remote consultation session, should email MEPA@mass.gov or the MEPA analyst listed in the Environmental Monitor. Requests for language translation or other accommodations should be directed to the same email address. Mail correspondence should be directed to the Secretary of Energy & Environmental Affairs, 100 Cambridge St., Suite 900, Boston, Massachusetts 02114, Attention: MEPA Office, referencing the above project.

READS Collaborative
Request for Proposals

Sealed proposals for custodial cleaning services at two locations for READS Collaborative. Contract will run for 3 years from 7/1/24 to 6/30/27 with 2 optional one year extensions. The two cleaning locations are 105 East Grove Street, Middleboro and 44 Bedford Street, Middleboro. The LAKEVILLE ZONING BOARD OF APPEALS, acting in accordance with MASS GENERAL LAWS CHAPTER 40A, as amended, will conduct a public hearing on Thursday, April 25, 2024, at 7:00 P.M. in the LAKEVILLE PUBLIC LIBRARY, 4 PRECINCT STREET, upon the petition of Carl Mason, Aerie President. A Special Permit is requested under Section 270-7.4 B and Section 270-7.4 B (21) for the demolition of an existing two-bedroom dwelling and the construction of a new two-bedroom dwelling of similar scale that would be within the setbacks. According to the provisions of Massachusetts General Laws, Chapter 131, Section 40 of the Wetlands Protection Act, there will be a hearing on a Notice of Intent submitted by Rich Adey, for the construction of a 12’ x 15.5’ covered porch and the resurfacing of an existing patio within 100’ of Assawompsett pond as well as within flood zone AE-57. The location of the project is 9 Twin Oaks Road, Lakeville (Map 065 Block 002 Lot 002). The public hearing will be held on Tuesday, April 23, 2024, at 7:00p.m. at the Lakeville Police Station, 323 Bedford St., Lakeville.
